1. PLI Policy no APS / NM / 5239-A having maturity date 03 Apr 2013 had been sent with all the necessary documents like original bond, form LI - 9(b) on 18 Mar 2013 through FPM 634 FPO C/O 56 APO (Chandigarh). But I have not received anything form your office till date.
2 . I request APS PLI Cell to make necessary arrangement for the maturity payment.

While undergoing basic training/serving at FS Coy, 1 TECH TRG REGT, 1 STC, Jabalpur (M.P) in year 20 Nov 1996 I opened PLI Policy a sum of Rs 60,000/-. And monthly premium is Rs 309/- is continuously deducted from my salary. Till this date I have not received PLI Policy document and Bond. Hence, it is requested that above document may please be forwarded to me at above address. A photocopy of may latest QSA / Pay Slip Dec 2012 is enclosed herewith for your further necessary action please.
