Credit card bill not waived

I had raised a complaint previously regarding my credit card not being delivered to me but me being charged a fee for it. In response the fee had been waived off along with a new card to be delivered to me. But the credit card bill is still available to me on the app and has not been waived off yet.
Also, the credit card is being delivered to Bangalore although my address in from Howrah, West Bengal.
Kindly look into the mater and help me out.

without intimation amount deducting

I am operating an S B A/C from last fifteen years after merging in ing minimum balance is 5000 and the informed and informed which services are chargeble. Now your people is deducted Rs 57-50 as ECS charged no bank is collecting the said charges.
Today I went to BHIMAVARAM branch no body is responded no complaint book. Manager is in phone and replied call will take two hours. I am wasted my one hour time in the branch. please infrom me what is chargeble services and what is minimum balance
I am not satisfied with your BHIMAVARAM branch service. atleast u reply me then i will close my account

UNAUTHORIZED MONEY DEBITED FROM MY BANK A/C

Sir,

This is to inform you that I hold a salary A/C in Kotak mahindra bank, (branch- Brabourne road, kolkata) bank and my CRN no is 50265615 and a/c no is 5611217717. I held this a/c under Mcnroe consumer product pvt ltd. On 1st of september, 2015at 5.44am rs 6191.36 was debited from my account and was credited in to the a/c of" WALGREENS CALUMET PARK US".Generally for every online transaction I received a dynamic access code on my registered mobile no and email id, but for this transaction I did not received that neither on my mobile no or email id. At around 10am on 1st september2015 I received a phone call from Kotak mahindra bank customer care no- 022- 66052624 about this transaction and the customer representative asked me to lodged a complain about this in Kotak customer care. I lodged a complaint and the complaint no is- 1-3805601643.

I requested you to do necessary action against this kind of misdeed.

I shall be highly obliged if you kindly do the needful as soon as possible.

Regarding false expectation for credit card

Regarding false expectation for credit card

Hi,
My Name is Amit Maharwar one of the account holder of your bank.
I have got the salary account with KOTAK MAHINDRA BANK.But,With
so much disappoinment i have to inform you that false expectations
were made to me because of which now am in a helpless situation.
On 8th-March-2012 on of your agent asked to me open a bank account,
I informed him that i have already got the salary account with kotak
but after that he asked me whether i have got the credit card or not,
i said no i haven't got then he informed me that you can apply for
it ,all you have to do is to give your documents{Salary Slip & pan card
xerox and a photograph} which i did after that he told me that you will
get a sms on your mobile regarding the application details however he
has not given me any application form to fill up,after ten days when i
called him to get the update he told me that he is not in the office and
will update me tomorrow and that tomorrow has never came almost more than
100 times i have called him but till date i have got any information,
after that i contacted Ajay MM regarding the same issue to check whether
what exactly is going on but Ajay also didn't show any interest in this
issue after all the hassle with a hope i visited the KOTAK MAHINDRA BRANCH
and they also didn't showed any interest in this and at that point of time
i was very frustrated and i said that i will cancel the account if this will
continue and they simply toldme about the cancellation procedure this what
customer service i have got from your bank which was never expected, my
sisteris going to get married on 15th-April-2012 andi was infact in need of
some financialhelp and if Avik{the agent who took my documents }would have
not misguided me at that time i would have arranged something for this butnow
I'm in a situation wherei can't do anything rather than regrating why i have
trusted on you, and with a lasthope I'm sending this e-mail to you so that
at least if you can't issue me a credit-card you can take some serious action
against these people who use to misguide people because of which they have to
